Reporting a Product Issue?

With each event, please include the following:

Pump serial number or administration set model & lot#.

If the event is related to an administration set issue, take a picture of the issue and infusion set up if possible. Put the administration set aside for evaluation.

Include a full description of the issue or difficulty encountered

List any known details on patient involvement, infusion settings, types of fluids, times, and alarm messages

Return the administration set used if possible
